Many people associate a hiatal hernia primarily with heartburn or acid reflux, but nausea is also a recognized part of the condition’s symptom profile. How a Hiatal Hernia Triggers Nausea
A hiatal hernia occurs when part of the stomach moves upward through the esophageal hiatus, the opening in the diaphragm through which the esophagus passes before joining the stomach. What Other Symptoms Can Accompany Nausea?
Nausea rarely appears in isolation when a hiatal hernia is the underlying cause. Most patients experience a cluster of symptoms that fluctuate depending on body position, meal size, and dietary choices. Common digestive symptoms include heartburn, acid reflux, epigastric pain, chest pain, burping, bloating, dysphagia (difficulty swallowing), and regurgitation of food or fluid into the mouth. Some patients also experience vomiting after larger meals or when they lie down too soon after eating.
The effects of chronic acid reflux are not always confined to the digestive tract. Some patients develop what clinicians describe as extraesophageal symptoms, including chronic cough, hoarseness, sore throat, and laryngopharyngeal reflux (LPR), in which stomach acid reaches the larynx and upper airway. Fatty meals, spicy foods, caffeine, alcohol, peppermint, and chocolate are widely recognized as contributors to reflux and nausea in patients with a hiatal hernia. Physical habits that increase pressure on the diaphragm should also be modified where possible: these include wearing tight clothing, engaging in heavy lifting, and straining during bowel movements, all of which can worsen symptoms in susceptible patients.
When to Seek Medical Attention
Persistent vomiting, difficulty swallowing, severe chest pain or epigastric pain, GI bleeding indicated by blood in vomit or black tarry stools, unexplained anemia, and a sudden or dramatic worsening of abdominal pain are all symptoms that warrant prompt evaluation. Although uncommon, a strangulated hiatal hernia, complete gastric obstruction, or gastric volvulus can restrict the blood supply to the stomach and constitute a surgical emergency.
